Pre Adoption Questionnaire

This pre adoption form will need to be completed on a PC or Laptop unless you have the have the word app on your phone or tablet. Please review the completion instructions ahead of submitting the form.

If you are interested in adopting Tyson, you will need to register your interest by filling in and returning one of our pre-adoption questionnaires.

If you live in rented accommodation, in addition to the completed pre-adoption questionnaire please include written permission from your landlord or housing authority, confirming that they are happy for you to keep a dog in the property. If you already have a dog/s the permission must state the number of dogs allowed – without this we cannot consider the application.

Completed application forms and landlord permissions if applicable must be emailed to and CC with ‘Tyson’ in the subject bar. Emails sent to other addresses will not be considered.

Tyson was brought into SDR as the owners were not allowed to take them into their new home.

Tyson would require an owner who has previous dog experience, but not necessarily of the breed.

Tyson is a little nervous when meeting new people, but once he has gained his confidence with you his playful nature comes shining through. Tyson loves nothing more than to chase tennis balls and will happily bring toys over to you for play.

Tyson walks beautifully on the lead, but can pull occasionally if he finds a nice smell he wants to investigate. He is not fazed by passing traffic when out on his walks, but loves to take in the surrounding fauna and flora. In addition to his walks Tyson would benefit from being able to let off some steam by going for a good run in a fully enclosed area a couple of times a week.

Tyson would need minimal training as he knows sit, down and wait but it would help to keep these routines up as he can be reluctant sometimes about doing them. He is also house trained.

Tyson may need his home alone time built up gradually as previous owner reported there were times he became unsettled when being left home alone.

Tyson has no specific dietary requirements, but when feeding him avoid fish based meats and treats as he does not like them and will leave them.

Tyson mixed well with other dogs at SDR, both on and off lead gauging what level of play the other dog was looking for. He played and ran around when mixed with another Lurcher who was also energetic, but was also calm and well mannered when mixing with a quieter dog. He could possibly live with another dog pending a successful dog meet at SDR.
